Complete before work begins. Retain with your job records and photographs.

Purpose: This document records the pre-existing condition of all surfaces before cleaning begins. It is your written protection against post-clean damage claims. Complete it thoroughly, take photographs, and have the client sign before any work starts. Do not begin work without a completed and signed copy.
